If you would like to make an appointment, please contact us by e-mail or by phone. We are available evenings and weekends to serve you. In preparation for your appointment, please print out one copy of the Client Information form and two copies of the Client Disclosure form (one is for you to keep). These forms can be found at the “forms” link below.

Read the disclosure form, fill out the information form, and bring both to your appointment. I will ask you to sign and date both forms upon your arrival.

Also, if you are being treated by a physician or other licensed health care practitioner (such as a chiropractor, psychologist, or therapist), please print out a copy of the referral form, fill it out, have your health care provider sign it, and bring it to the appointment with you.
